Welcome! Our mission is to partner with families in helping children know, love, and follow Jesus Christ through biblical teaching, caring relationships, and a safe, Christ-centered environment.

LIABILITY WAIVER AND RELEASE FORM FOR KIDS MINISTRY ACTIVITIES

I, the undersigned parent or legal guardian of the student being registered, hereby grant permission for my child to participate in the activities organized by Sulamita Church. I recognize that the activities may involve physical activity and interactions that may pose risks to my child. I understand that Sulamita Church takes reasonable steps to provide a safe environment for all participants and that unforeseen incidents may occur.
I agree to release, indemnify, and hold harmless Sulamita Church, its officers, directors, employees, agents, and volunteers from any and all claims, liabilities, or lawsuits arising from my child's participation in these activities, including those caused by negligence.

I give permission to Sulamita Church to seek emergency medical treatment for my child if necessary. I agree to be responsible for any medical or other charges in connection with my child's participation in the activities.

I have read this release and waiver of liability, fully understand its terms, and sign it freely and voluntarily.

MEDIA RELEASE FORM

I give Sulamita Church the permission to photograph my child and use his or her picture or video solely for the church's use, website, printed materials, or social media. Sulamita Church will never publish a child's name with any of its publications.
